When Irfanview is opened via an association, the menu bar is off a level. In other words, if you highlight File, the tooltip shows Slideshow, and clicking on file takes you to the slideshow menu. Not sure this is Vista only, but I don't recall having this problem on XP.

I think you mean the tool bar, not the menu bar. Try changing to a different toolbar. Perhaps there is an error in the toolbar skin.
